1. Visit the "BOOK ONLINE" page

2. Select your preferred date and time to attend clinic.

3. Fill out the information form + provide us with all information possible.

This will help us best serve your player!

4. At the bottom of the form there is a payment section. Select a payment plan from the drop down menu.

5. Click the "buy a plan" button on the right hand side of the screen

​

6. To complete your purchase, click the "sign up" button to create a log in and password for the website. This will save your information and allow you to bypass the booking form each time you reserve a spot for clinic.

7. Enter your payment information and click the "buy now" button to complete the registration process.
